Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that:
a)  Hydro-San Inc.  employees working at Kit Carson Street and
Clarkson Street, LIttleton Co:  on or about 10/18/91 employees
were exposed to potential oxygen deficient atmosphere as well as
potential exposures to hydrogen sulfide and methane gas while
entering sewer lines without benefit of comprehensive confined
space entry procedures in that:
Elements such as, but not limited to, the following were not addressed:
1.  A confined space entry permit system was not in place.
2.  Atmospheric testing was not done prior to confined space entry.
3.  an emergency response rescue program was not in place.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to implementation of a confined space entry
program in accordance with ANSI Z117.1-1989.

a)  Hydro-San, Corp. employees working at Kit Carson Street and
Clarkson Street, Littleton, CO: on or about 10/18/91 employees
were exposed to the hazard of being struck by oncoming vehicles.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to provide adequate traffic diversion,
including the use of traffic signs and adequate taper with traffic
cones in accordance with Section 6C of ANSI D6.1-1971 (Manual on
Uniform Traffic Control Devices for Streets and Highways).
