Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazard of being crushed by refuse collection trucks; 1984 chevrolet, 1979 chevrolet, 1976 GMC and 1977 GMC which were not equipped with an external audible warning signal device which activates when the vehicle is operated in reverse.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazard of being crushed by dumpster due to the 1/2 inch wire rope running line on the 1984 Chevrolet Model 120 loadmaster having an inadequate number of cable clips and these cable clips positioned with U-bolts with live end of the rope.
