Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazards associated with a fall from an unstable aerial lift work
platform:
a) Home Depot Shopping Mall, Rt. 110; Employee with the job title of
Steamfitter working on the platform of a motorized aerial work lift,
Sylvan Sizzor J.L.G. 23 M.S., s/n 502600, installing a sprinkler
system approximately 22 feet above the ground. The employer allowed
the employee to work on the aerial lift with a completely deflated
tire.This condition created an uneven work surface from which the
aerial lift was operated and compromised its stability. On or about
10/10/90.
b) Home Depot Shopping Mall, Rt. 110; Employee with the job title of
Steamfitter working on the platform of a motorized aerial work lift,
Sylvan Sizzor J.L.G. 23 M.S., s/n 502600, installing a sprinkler
system approximately 22 feet above the ground. The employer allowed
the employer to work on the aerial lift without the outriggers
extended. The operation of this equipment with the platform in an
elevated position, without the use of the outriggers, changes the
aerial lift's center of gravity and compromises its stability. On
or about 10/10/90.
ABATEMENT NOTES:
Among other methods, one feasible method to reduce this hazard is to
establish and enforce adequate safe operating procedures. Procedures
such as those stipulated by the manufacturer's brochure "Platform
Operation and Safety Handbook", and to follow the safety guidelines
stipulated in American National Standard for Self-Propelled Elevating
Work Platforms. (ANSI A92.6 - 1979). Elements essential to an acceptable
safe operating procedure include at a minumum:
1) All tires be properly inflated.
2) Outriggers are set before elevating platform in accordance with the
equipment manufacturer's specifications.
