correct the hazard is:
1) Start implementation of the existing written policy pertaining to the
coupling of yard-birds (tractors) to trailers.
2) Improved training of all yard shifters in the existing written
trailer coupling policy.
3) Improved supervision and enforcement of existing written trailer
coupling policy.
