Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serius physical harm to employees in that employees were exposed to: a) The hazard of fire and explosion due to the gas propane stove located in the doghouse. Among other methods, one feasible and acceptable method to correct this hazard is to equip the doghouse with a non-spark producing stove approved for class I division 2 locations.

Among other methods, one feasible and acceptable method to correct this hazard is to locate the blooey line at least 150 feet from the wellhead, and downwind or at a right angle to the prevailing wind direction.

Among other methods, one feasible and acceptable method to correct this hazard is to install a anti-two-blocking device that would prevent accidental two-blocking of the traveling block and crown block.
