Section 5(a)(1) of the Occupational Safety and Health Act of 1970: On
or about 7/5/88 the employer did not furnish employment and a place of
employment which were free from recognized hazards that were causing or
likely to cause death or serious physical harm to employees in that:
a) Employees erecting structural steel columns and beams at 444 E.
Henrietta Road, Monroe County Pure Waters Operation and Storage
Facility were exposed to falls of approximately 20'4" to the ground.
On 7/5/88, employees engaged in connecting two columns and a beam
when it toppled to the ground causing the employees to suffer broken
bones and severe back injuries. Contributing circumstances were the
improper shimming of the columns with a concrete brick.
Among other methods, one feasible and acceptable method of abatement is
to tighten down the anchor bolts before cutting loose of the crane, as
stipulated by ANSI A10.13-1978, Safety Requirements for Steel Erection,
Subpart 10, Section 10.1.10

a) Employees erecting structural steel framework at 444 E. Henrietta
Road, Monroe County Pure Waters Operations and Storage Facility were
exposed to falls of approximately 20'4" to the ground from steel
framing. The framework was not properly braced to enhance the
structural integrity of the structure.
Among other methods, one feasible and acceptable method of abatement
is to stiffbay the framework. Stiffbaying would require the tie joist
at the column lines to be welded at both ends as stipulated by ANSI
A10.13-1978, Safety Requirements for Steel Erection.
