Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The

employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
The hazard of collapse/toppling of the meat (derrick) of a well
service rig resulting from inadequately installed guyline anchors,
in that the pull out integrity was not determined to assure adequate
anchorage. This condition existed May 20, 1987, on J & C Drilling
Co., Rig #10, at the O'Connor Well #68, 22 miles North of Refugio,
Texas, while employees were required to perform work off the rig
floor and monkey board.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to properly install guyline anchors and ensure
pull out integrity for adequate anchorage by conducting pull tests.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
Employees were exposed to the hazard of well blowout and fire, or
other emergency, while on the mokey board.
Among other methods, one feasible and acceptable method to protect
employees would be to install a properly rigged and secured escape line
(Geronimo line) or other satisfactory auxiliary means of escape from the
derrick. The condition occurred on or about May 19, 1987, 22 miles North
of Refugio, Texas, O'Connor Lease Well #68, employer's Rig #10.
