Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards what were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of being crushed by the platen of the thermoforming machine
when employees are required to perform maintenance work inside the press
bed, due to accidental cycling of the controls while the power to the
press is on, or by loss of air pressure to the pneumatic cylinders which
hold up the platen:
a) Production Department. Thermo-forming Machine, Model 95, SN-122.
Among other methods, one feasible and acceptable abatement method, is to
lock out the electrical power supply to the machine and provide positive
stops under the platen when working inside the press.
