Section 5(a)(1) of the Occuaptional Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed to:
On or about 10/2/87:
a) The hazard of being crushed which could result in serious physical
harm or death. Employee, who was removing outer eccentric bushing
from #7 Simons stone crusher, S/N 7399, main frame, while the main
frame was top side-down, was exposed to being crushed by the bushing
after the locks were removed, which permitted the bushing to drop out
of the bore.
One feasible and acceptable abatement method, among others, is to remove
the bushing while the main frame is situated in the top side-up position
as required by the manufacturer's instructions.
