Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about November 23, 1989 employees working in boiler 4 building basement were exposed to the hazard of flying metal and steam burns created by the blowout of an undersized relief valve which was located between turbine discharge exhaust and the first main stop valve on the exhaust discharge side of turbine No. 1. Among other methods, one feasible and acceptable abatement method to correct this hazard is to upgrade the size of the existing turbine exhaust outlet relief valve for the No. 1 feed water pump and to conform with standard recommended engineering requirements of the most recent A.S.M.E. boiler and pressure vessel code, section VIII; in accordance with requirements of paragraphs UG-125 through UG-134.

Among other methods, one feasible and acceptable abatement method to correct this hazard is to install a properly rated relief valve in the system between turbine exhaust discharge and the first main stop valve on the exhaust discharge side of turbine No. 4 and to conform with standard recommended engineering requirements.
