Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by concrete slump and becoming incapacitated without immediate medical attention while engaged in chipping concrete from interior walls of concrete mixer barrel. On or about 3/13/89. a) Employees were exposed to hazard of being struck by concrete slump and becoming incapacitated without immediate medical attention while engaged in chipping concrete from interior walls of concrete mixer barrel. On or about 3/13/89. Among other methods one feasible and acceptable method to correct this hazard is to: Implement a confined space entry procedure as required by ANSI Z117.1-1977 which includes but is not limited to the following items: 1- Posting of written confined space entry procedures adjacent to or on mixer barrels of concrete trucks which may be entered. 2- Implementation of established lockout procedures - Employee working inside mixer barrel maintain sole ignition key on his person. 3- Provisions for a designated person to communicate and observe employee working inside mixer barrel as means of obtaining immediate help in an emergency. 4- Provisions and procedures for readily available medical assistance. 5- Implementation of periodic training sessions relative to confined space entry, rescue and first aid procedures.
