Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which was
free from the recognized hazard of potential oxygen deficient atmosphere
/or inhalation of toxic gases or vapors in a confined space:
a) Employees entering frac tanks located at a gas well known as Hansell
Well Number Two just off FM Road 31 approximately ten miles southeast
of Marshall in Harrison County, Texas.
The hazard could be corrected by methods such as providing air supplied
respirators, testing confined space atmosphere before entry, proper
forced air ventilation of the confined space, adequate training of
employees or by following procedures outlined in American National
Standard Institute (ANSI) Standard Z117.1 "Safety Requirement for
Working in Tanks and Other Confined Spaces". (Copy enclosed)
