Double nine:  Employees are exposed to the possible death or serious
physical injury in that, the reverse signal alarm was not audible
above the surrounding noise level while loading and unloading sand
with the Komatzu 500, WA500, 13-613 Loader, and no observer was
present to signal the operator.
Among other methods, one feasible and acceptable abatement method to
this hazard is to:
1.  Ensure the back-up alarm is functioning properly before each
shift and periodically thereafter.
2.  Provide a cover to eliminate the build-up of sand into the
alarm.
