Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a.The employer failed to follow their own written procedure for Tag Out, Section 7.1.3 in that only the person requesting the Operating Permit (Green Tag) went to the second floor of the Electrical Control Building for the SO2 Removal Project to inspect the equipment prior to, and to hand the operating tag, on the controls for Power Center SCA-16 and SCB-16. Among other methods to correct this hazard would be to follow the APS Bulletin 54 Rev. 10 of April 1993.
