Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
electric shock, arc and burn hazards.
A) R-1 386 Mill (130 inch mill) Thyristor SCR Cabinets - The employer
failed to write an adequate procedure for employees performing
"dummy load tests" on silicon control rectifiers in that safety
procedures, safety equipment to be used and safety precautions to
take were not described in the "#386 Dummy Load Power Check for
Thyristor Power Cases, Main Mill" procedure, on or about July 19,
1990.
Feasible and acceptable abatement methods, among others, include writing
a procedure that discusses the hazards, what safety precautions to take,
the type of safety equipment to use and a job safety analysis for the
sequences and other hazards that one may confront employees during the
testing.

employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
of seroius physicl harm to employees in that employees were exposed to
electric shock, arcs and burn hazards:
A) R-1 at Thyristor SCR Cabinets for 386 Mill (120 inch Mill) -
electricians performing dummy load tests on Thyristor SCR
power cases were unqualified and untrained for the testing
procedures being conducted on energized electrical parts
rated at 630 volts AC and having the potential to phase
over and triple the voltage on or about July 19, 1990.
Feasible and acceptable abatement methods , among others, is to develop
and implement a training program tailored specifically to hazardous
non-routine job tasks that expose employees to high voltage hazards.
The program shall list at a minimum the skills, techniques and sequences
that must be maintained during specific tasks and the employer shall
develop the program to provide "hands-on training skills" through use
of diagrams or simulated wired equipment. The program shall describe
all the safety procedures for specific hazards and any precautionary
techniques, personal protective equipment, insulating or shielding
materials to be used.

or serious physical harm to employees int that employees were exposed to
electric shock, arc and burn hazards.
A) R-1 386 Mill (130 inch Mill) at SCR Cabinet Banks - in preparing
to perform a dummy load test, employees were exposed to energized
electrical parts rated at 700 volt AC while connecting test lead
cables to 700 volt DC posts located within an inch of the AC posts
and cables, on or about July 19, 1990.
Feasible and acceptable abatement methods, among others, is to retrain
supervisors and technical personnel acting as supervisors in using the
lockout/tagout procedures and reiterate to them that they have the
responsibility to see that lockout/tagout procedures are to be followed.
