free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
burns:
A) Shop Area - From the ignition of a leak or damage to the rubber
hose supplying natural gas to a portable heater with no cut-off
valve in near proximity to or at the area where the rubber hose
connects to the metal gas line, on or about November 29, 1990.
Among other methods, one feasible and accepted means of abatement to
correct this hazard is to install a cut-off valve at the rubber hose
metal line connection, and locate shut-off valves as stipulated by NFPA
54-84 (ANSI Z223.1-1984) Natural Fuel Gas Code.
