Section 5(a)(1) of the Occupational Safety and Health Act of 1970: On
or about 11/17/87 the employer did not furnish employment and a place
of employment which were free from recognized harards that were causing
or likely to cause death or serious physical harm to employees in that
employees were exposed to:
a) The hazard of falling appeoximately 125/135 feet into water while
painting bridge steelwork form a stage/platform of a Catenary
scaffold. The stage/platform had no ties to its supports to prevent
it from slipping off the wire rope support on one end, or the catwalk
rail support on the other end.
One feasible and acceptable abatement method among others to correct
the hazards, is to have hook-shaped stops or sliding slings to connect
platform to cable and platform to catwalk rail, as stipulated in Local
Union #806 Working Rules, paragraph #30, and American National Standard
A10.8-1977 section 22 paragraph 22.1 for Catwalk Scaffolds.
Location: Span #15 of Driscoll Bridge.

or about 11/17/87 the  employer did not furnish employment and a place
of employment which were free from recognized hazards that were causing
or likely to cause death or serious physical harm to employees in that

a) The hazard of falling to ground or into water while climbing from
catwalk to the stage/platform of a catenary scaffold while painting
bridge steelwork.
One feasible and acceptable abatement method among others, to correct
this hazard is to wear a satety belt with lanyard already attached to
an overhead safety lifeline while climbing onto the stage/platform on
the Catenary scaffold.
As stipulated in American National Standard Safety Requirement for
scaffolds A10.8-1977, Section 22 Catenary Scaffolds, paragraph 22.7 and
New York Industrial Code Rule 23 Administration Resolution No. 127,
paragraph #9.
Location: East Catwalk of Driscoll Bridge.

a) The hazard of falling to ground or into water while painting bridge
steelwork from a stage/platform of a Catenary Scaffold. The lifeline
(running overhead and crossing the stage/plaform) lies across the top
of bridge support beams and was not protected from chafing, or
abrasion when employees were tied onto lifeline which moves from side
to side along the beams.
One feasible and acceptable method among others, to correct this hazard
is to protect the cable with padding, wrapping and chafing gear of lower
or non-friction material (i.e., wood-cloth-leather-fiber) as stipulated
in #27 of Union local #806 Work Rules; 23-1.16(e) of New York Industrial
Code Rule 23, and 4.33.2.4 of New Jersey Construction Safety Code.
Location: Catenary Scaffold, East Side of Driscoll Bridge.

this hazard is for the wire rope to be continous without splices between
anchors as stipulated by American National Standard A10.8-1977 22
Catenary Scaffolds, paragraph 22.2 and New York State Industrial Code
Rule 23-5.1., paragraph (k) splices.
Location: Wire rope support at Span #21 approximately 20 feet south of
pier #21-Driscoll Bridge.
