Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of inhalation of cobalt dust which can lead to cumulative lung disease (hard metals disease), asthma and dermatitis: a) Baystate Carbide Products Inc., Machine shop - Workers involved with grinding tungsten carbide were exposed to cobalt dust on March 2, 1990, March 6, 1990, and April 4, 1990 without protection by a medical monitoring program to protect workers against the early onset of cumulative lung disease (hard metals disease), asthma and dermatitis (see descriptions of operations involving exposure to cobalt in citation #1, Item 3b and 5b and citation #2, Items 1b and 2a). Among other methods, one feasible and acceptable abatement method to correct this hazard would be to require all workers exposed to cobalt levels greater than or equal to 50% of the permissable exposure limit (PEL=0.05 mg/m3) to be included in a medical monitoring program. ABATEMENT NOTE a medical monitoring program should include at a minimum: 1) A baseline chest x-ray (14" X17" chest roentgenogram). 2) Complete medical history. 3) FVC and FEV (1 sec) pulmonary function test. 4) The physical exam should include complete evaluation for determination of employee capability to wear respiratory protection. 5) All medical test results will be discussed with the worker by the physician. Medical follow-up program should include the following procedure: 1) Pulmonary function tests annually and chest x-ray once every 3 years. 2) A worker shown to be developing pneumoconiosis should be removed from exposure. 3) Employment termination physical including chest x-ray pulmonary function test.
