Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of entering a potentially heat stressfull environment and
oxygen deficient atmosphere without a means of rescue in an emergency:
a) Incinerator Area - The incinerator operator who periodically has to
enter the incinerator to remove melted glass from the air vents on
or about 5/19/89 did not have a means of emergency rescue in the
event that the atmosphere within the incinerator were to deteriorate
or the operator was to be overcome by the heat.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to use a safety belt and lifeline with a trained
stand-by person stationed outside the incinerator in an emergency, such
as described by ANSI Z117.1-1977, Sections 3 and 4, Safety Requirements
for Working in Tanks and Other Confined Spaces.
