General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of falling into the hoistway shaft when the elevator car was not in the landing zone. Among other methods, one feasible and acceptable method to correct this hazard is provide an elevator hoistway door or gate that is electrically interlocked so that latched doors or gates cannot be opened from the landing: Freight Elevator, 2nd and 3rd Floors - A means was not provided to prevent employee exposure to falls of up to sixteen feet to the 1st floor, in that the sliding doors on the 2nd and 3rd floors could be opened without the elevator car being at the landing.
