Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to inhalation hazards from possible leaks, or releases from the system's, pipelines, vessels, valves and equipment handling anhydrous ammonia; The following condition contributed to the existence of these hazards: Refrigeration Area - The preventive maintenance program for inspecting critical valves was not adequate. ABATEMENT NOTE: The program should include visual and thickness testing of ammonia valves and piping, evaluation of piping for corrosion, erosion, proper support, vibration, and replacement.
