Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to injuries resulting from a peroxide explosion and/or fire:
A. RUBBER COMPOUNDING/WEIGHING AREA:
1. CONTAINERS OF ORGANIC PEROXIDES WERE STORED IN THE AREA WHERE
RUBBER INGREDIENTS WERE MIXED.
B. MIXING ROOM BUILDING 7:
1. CONTAINERS OF ORGANIC PEROXIDES WERE STORED UNDER THE MIXING
BENCH NEAR WHERE FLAMMALBE LIQUIDS WERE STORED AND PROCESSED.
C. SOUTHEAST CORNER BUILDING 2 NEAR COATING MACHINE #3:
1. CONTAINERS OF ORGANIC PEROXIDES WERE STORED AT CHEMIST'S BENCH
NEAR WHERE FLAMMABLE LIQUIDS WERE STORED AND USED.
D. COLD BOX (PEROXIDE STORAGE COOLER):
1. BULK QUANTITIES OF PEROXIDES WERE STORED IN THE COOLER ALONG
WITH FLAMMALBE SOLIDS.  ELECTRICAL WIRING INSIDE COOLER WAS
NOT EXPLOSION PROOF.  THERE WAS NO EXPLOSING VENTING.  THE WALLS
OF THE COOLER WERE NOT BLAST RESISTANT.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD
TO CORRECT THIS HAZARD IS AS FOLLOWS:
1. LIMIT QUANTITIES OF PEROXIDES IN PROCESS AREAS TO ONLY THAT
AMOUNT NECESSARY FR IMMEDIATE NEEDS.  STORE ADDITIONAL QUANTITIES
IN ACCORDANCE WITH NATIONAL FIRE PROTECTION ASSOCIATION - CODE
NO. 43B - "STORAGE OF ORGANIC PEROXIDE FORMULATIONS."
2. STORE BULK QUANTITIES OF PEROXIDES IN ACCORDANCE WITH SECTION 2-11
"STORAGE LIMITATIONS" OF NFPA 43B.  STORE PEROXIDES SEPARATE FROM
FLAMMABLE SOLIDS.
