Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
Employees were exposed to the hazard of death by electrocution in that
the lockout/tagout program was deficient as the journeyman electrician
in charge of the trainee electrician who was working on the Butcher
Boy bandsaw B.B.15 in the strip department on 6/25/87 did not have his
own padlock in place on the power disconnect for the saw, didnot
inspect the trainee's work before the circuit was energized, and did
not require employees to stand clear of the saw when it was energized
for testing purposes.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is:
1) Require all employees involved in repair and maintenance activities
to use their own padlocks when a lockout/tagout is needed.
2) Requires electrical work to be inspected by a journeyman electrician
prior to energization of circuits.
3) Requires all employees to stand clear of equipment which is being
energized for testing purposes.
