Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees, in that employees were exposed to
the hazards of having part of their body caught between the moving
elevator and the hoistway.
A feasible and useful method to correct this hazard, among other methods
is the installation of elevator car doors or gates that would prevent
employees from having any part of their body outside the periphery of
the elevator car unless the elevator car is within the landing zone and
that were electrically interlocked to prevent the elevator car from
moving unless the elevator car doors or gates are in the closed position
a) Building #3, mill room:  The freight elevator which runs between the
1st floor and mill room balcony, aproximately 10' rise did not have
walls nor interlocked gates.
b) Building #5, clear room:  The freight elevator which runs from 1st
floor to mezzanine, 7-1/2' rise, and to top landing, 11-1/2' (total)
rise, did not have walls nor interlocked gates.
c) Building #6, color room:  The freight elevator which runs between the
1st floor and mezzanine approximately 11-1/2' rise did not have walls
nor interlocked gates.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees, in that, employees were exposed
to the hazards of falling down the hoistway shaft when the elevator car
was not in the landing zone, being struck by the elevator car when the
car was entering or leaving the landing zone or having part of their
body caught between the moving elevator and the hoistway.
A feasibleand useful method to correct this hazard, among other methods
is to provide elevator hoistway or landing with doors or gates to
prevent employees from having any part of their body within the hoistway
shaft, outside the periphery to the landing, unless the elevator is
within the landing zone and that were electrically interlocked to
prevent the elevator car from moving when the hoistway or landing doors
or gates are in the open position and so latched that the door or gates
cannot be opened from the landing side unless the elevator car is within
the landing:
a) Building #3, mill room:  The freight elevator, which runs between the
1st floor and mill room balcony, approximately 10' rise had two (2),
landing gates on the 1st floor approximately 38" high, none of which
were interlocked.
b) Building #5, clear room:  The freight elevator, which runs from 1st
floor to mezzanine and to top landing, 11-1/2' (total) rise, had one
landing gate on each level approximately 28" to 32" high, none of
which were interlocked.
c) Building #6, color room:  The freight elevator which runs between the
1st floor and mezzaine, approximately 11-1/2' rise, had landing gate
at mezzaine level, aproximately 36" high, and gate at 1st floor level
approximately 40" high, neither of which were interlocked.
