General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
possible electric shock:
a) the 440 volt disconnect for vat #2 in the "make" room was not locked
out per company procedure when the agitator motor was removed and
because of this situation the bare leads for the motor were only
protected by a start/stop switch.
Among other methods, one feasible and acceptable abatement is to ensure
that disconnects are locked out before and during any work on equipment
that could create a hazard should it become inadvertently energized.
