Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of being struck or run over by the wheels of travelifts
traveling in the forward or reverse direction where employees walk and
work in close proximity to those wheels during plant operations:
a) On February 10, 1987 and February 12, 1987, at the north section of
the facility, in the pre-cast storage area, where Drott Travel-lift,
company numbers 101, 102, 104 and 105 were being used.
Among other methods, one feasible and acceptable method to correct this
hazard is:
1) To equip all wheels including wheels hidden from the operators view
with personnel deflecting type wheel guards, jumpers or skirts; and,
2) Provide automatic audible alarms distinguishable above the
surrounding noises to assure that all employees are clear of the
machine before the operator begins travel in either direction and
during travel; and,
3) Provide revolving lights or beacons operational when the machine
initiates travel and during travel in either direction; and,
4) Provide a continuing training program for employees operating
travelifts or working in the vicinity of this equipment.
