Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: a. Multiple injuries from fall hazards associated with two employees walking the web of beam 11B6 from the secured west end to the unsecured east end to make the connection to Column 1CZ; as per ANSI A10.13-1972 Section 8.2, when connectors are working in pairs, one end of steel shall be bolted before going out to connect the other end, and then only one connector shall go out to fasten the other end, on or about July 8, 1993. Among others, one feasible means of abatement would be to have one employee on steel to make connection at un-bolted end.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: a. Multiple injuries from fall hazards while working on Column IC2 which was setting on 2 bolts with leveling nuts in the column web, with the column base being approximately 1 3/4 inch from the concrete pier, and out of plumb approximately 1.77 ft. with the existing pipe rack pier where the 40 ft. beam #11B6 would be connected; was not adequately supported by the use of shims between the column base plate and the concrete pier or by the use of guy lines, or by installing beam #13B4 as a bent prior to connecting beam #11B6 to Column IC2, on or about July 15, 1993. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: a. Multiple injuries from fall hazards while attempting to install beam 11B6 to column IC2 in that the employer did not implement the field modification of beam 11B6 prior to installaiton, to compensate for th 1.77 ft. off set of the existing pipe rack column connection point of the beam west end, as recommended by the Ashland Peteroleum Co. Project engineer on 6/23/93. One feasible means of abatement would be to implement the field modifications as recommended by the Ashland Petroleum Company Project
