C) Written maintenance procedures are not written in a maintenance program manual, on or about August 22, 1990. The following conditions contributed to these hazards existing: A) The company does not have a program to systematically inspect and maintain storage vessels so that potential defects may be detected and corrected before an emergency arises, on or about August 22, 1990. B) Wall thickness inspections are not conducted every two years and logged for the predesignated exterior area of tanks. No records are available showing tests being done, on or about August 22, 1990. C) Records are not available showing that hydro static and visual test of tanks has been done, on or about August 22, 1990. D) Wall thickness inspections and records are not done on a six year basis, along with a visual interior inspection for each tank. Barge storage tanks #1, 3 & 4, 300,000 lb. capacity chlorine storage receivers #1, 1, 3 & 4, on or about August 22, 1990. Feasible and accepted abatement methods, among others, include developing a preventive maintenance and inspection program for tanks storing chlorine. Program should include wall thickness inspections, interior inspections and hydrostatic testing every 2 to 6 years. Records of inspection and maintenance work shall be recorded in same manner for review and historic information.

The following conditions contributed to these hazards: A) Pressure relief device was installed to permit introduction and retention of scale and debris into the discharge outlet flange which pointed upwards. This could prevent full relief throttle and overpressurize the steam evaporator, thus exposing employees to sodium hydroxide and flying debris, on or about May 8, 1990. B) Relief setting for device is unknown, on or about May 8, 1990. C) Relief setting is not marked on the pressure relief device, on or about May 8, 1990. D) The relief devices vessels MAWP is unknown, on or about May 8, 1990. E) Relief device is not readily accessible, on or about May 8, 1990. F) Markings are not on valve per UG-129-(a), on or about May 8, 1990. Feasible and accepted abatement methods, among others, include the following: A) Install relief device to prevent introduction and retention of debris. B) Ascertain relief valve setting and retain record of the valve history. Setting should be based on the MAWP of the host tank or vessel. The orifice size of the relief device has to prevent pressure from rising more than 10% above the MAWP. C) Install relief device to make it readily accessible. D) Retain markings on devices according to ASME code UG-129.

The following condition contributed to the existence of the hazards: A) The maximum allowable working pressure (ASME require MAWP) for the chlorine barge storage vessels is 225 psig and their pressure relief devices are set at 375 lbs per square inch. CL2 tank #1, CL2 tank #2, CL2 tank #3 and CL2 tank #4, on or about August 22, 1990. B) Numerous pressure relief devices did not have markings stating the manufacturer or identifying tradmark, the manufacturer's design or type number, size, capacity in pounds of saturated steam per hour or cubic feet of air per minute, pipe size of the valve inlet and set pressure, on or about August 22, 1990. C) A preventive maintenance program is not utilized to service various relief devices to assure their reliability, on or about August 22, 1990. Feasible and accepted abatement methods, among others, include having a master list of pressure relief devices and to what vessel they service identifying the relief devices and assuring section viii UG-129 (markings) of the ASME code is met, and placing the relief devices on preventive maintenance programs to assure they are inspected and tested according to services and ASME codes.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to inhalation and skin hazards from possible breach of containment of chlorine gas from pipelines. The following conditions contributed to the existence of these hazards: A) Welding on pipelines is not documented, on or about August 22, 1990. B) Welders certifications under Section ix of the ASME Code are not available, on or about August 22, 1990. C) Proof of x-rays and welds is not available or done for 100% of welds made on pressurized piping (15 psig or >), on or about August 22, 1990. Feasible and accepted abatement methods, among other, include developing a program on welding that requires welders be certified for specified welds and certification be available for review; that welds be documented for work and that x-rays of welds be performed and records retained.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees working in cooling and drying and at chlorine barge storage rack areas are exposed to inhalation/explosion hazards. The following contributed to the existence of these hazards: A) 300,000 lb capacity storage tanks containing chlorine had quick opening valves used for isolation of chlorine storage tanks #1, 2, 3 and 4. The valves are not rendered inoperable by locks or other means to avoid inadvertent opening of the valves which open completely with a 90 degree rotation of the level handle, on or about August 21, 1990. Feasible and accepted abatement, among others, to correct the hazard is to develop a program that contains and describes policy, procedures and control to car seal or lock quarter turn valves in the open position when they are relied on to permit venting or to block venting for operational considerations, on or about August 21, 1990.

The following conditions contributed to the existence of this hazard: A) No maintenance or preventative maintenance program, on or about May 30, 1990. B) Only one limit switch was provided in lieu of two, on or about May 30, 1990. C) No sign at top floor warning user to get off, on or about May 30, 1990. D) No warning light provided to warn of top floor, on or about May 30, 1990. E) Gates at various floors were left open and not self closing type. Gates were found left open, on or about May 30, 1990. F) Toeboards were not provided at manlift hole, on or about May 30, 1990. G) Various components of the manlift are excessively corroded, on or about May 30, 1990. Feasible and accepted abatement methods, among others, include the following: 1) Place the manlift on a preventative maintenance program that would include frequent inspections. 2) Add another limit switch to the manlift. 3) Place signs at top floor warning the operator to get off. 4) Place a warning light on the manlift at top. This is to warn operator to get off. 5) Install a device that would automatically close gate to manlift. 6) Install toeboards at manlift holes. 7) Repair and paint corroded parts of manlift.

B) Cell Building - Jib hoist, Yale manufacturer, 4,000 lb capacity had a hook that would not swivel, the hoist cable would not wrap on the drum properly, and the limit switch was not being tested each day, on or about June 12, 1990. C) Welding Shop - Underhung trolley hoist had a stretched hook that indicates the crane is being overload and stretched hook is indicative of poor inspection and maintenance, on or about June 12, 1990. Feasible and accepted abatement methods, among others, include the following: 1) Coffing Hoist - Place a strain relief device on the pendant. 2) Jib Hoist, Yale manufacturer - a) Replace or repair the swivel of hook. b) Repair the hoist to allow the rope to wrap correctly. c) And instruct all employees to test the limit switch before each shift. 3) Replace the hook for the welding shop underhung trolley hoist.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazard that were causing or likely to cause death or serious physical harm to employees in that employees working at various locations including, but not limited to, Rack #1, at posts 13, 14, 8, 9, 10, 18 and 20, salt building at conveyor room where some floor areas were totally corroded away, beam support at 70% caustic-north end, caustic building rack, and numerous other tanks and equipment supports at chloralkali and waste water are exposed to crushing injuries and inhalation hazards: The following conditions contributed to these hazards: a) Numerous pipe and rack supports are severely corroded. b) Vessel corrosion is noted throughout plant. c) A structural assessment of the plant was done by an engineering firm. Noted throughout the report are deficiencies listed as "poses a safety concern, immediate attention needed". No remedial action plan to address these hazards has been implemented stating the time frame and schedule for completion. Feasible and accepted abatement methods. among others, include implementing a plan to correct the numerous deficiencies identified by the engineering report and establish goals for completion.
