Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the inhalation and skin absorption effects of Methanol: A) Methanol Tank Storage Area/TDA Tank Farm - Due to the fact that there were no level indicators at the Methanol Tank Storage Area for tanks #001-037 and #001-029 to be used as a back-up for the level indicators in the control room, on or about January 10, 1991. A feasible and useful means of correcting the hazard would be to install instrumentation so that levels can be monitored at the tank farm as well as from the control room.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the inhalation and skin absorption effects of Methanol: a) Panel Board/TDA Control Room - due to the fact that the high/low level alarm indicator light in the TDA Control Room was defective for Methanol Tank #001-037, on or about January 10, 1991. A feasible and useful means of correcting the hazard would be to test the alarms on a regular basis and repair and replace defective bulbs to keep them operative at all times.
