Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
a)   The employer did not furnish employment and a place of
employment which were free from recognized hazards that were causing
or likely to cause death or serious physical harm to employees in
that employees were exposed to crushing, and other trauma injuries
associated with loss of load resulting from employer's failure to
provide operators with manufacturer's operating instructions and
failure to train employees in the proper inspection, operation and
safety precautions associated with the use of the shop crane.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is:
1.   Obtain operating instructions for the floor crane, place warning
signs on the crane pointing out the load limitations, proper
positioning of loads, hazards of tipping on uneven floors and other
operating instructions published by the manufacturer/supplier and/or
ASME/ANSI Pald 12 and updates.
2.   Replace with other mechanical assists such as, but not limited
to, overhead cranes, forklifts, and other material handling devices
that can be safely employed.

death or serious physical harm to employees in that employees were
exposed to:
a)   The employer did not furnish employment and a place of
employment which were free from recognized hazards that were causing
or likely to cause death or serious physical harm to employees in
that employees were subjected to falls, pinchpoint and other crushing
hazards while unloading aircraft engines using the hydraulic Waltco
lift.  Some unsafe work practices related to the exposure include:
a)   Riding on the hydraulic lift gate to gain access to
the truck bed and exposing themselves to the pinchpoint
when the gate closes against the truck bed.
b)   Using liftgates whose width does not ensure stability
because loads are wider than the supporting area.
c)   Employees standing in the fall zone with unstable
loads.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to follow the manufacturers instructions
and warning signs for safe operation of hydraulic lifts.
