free from recoginized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) On or about September 15, 1989 employees were exposed to the hazard
of using an unreliable pressure gauge when engaged in the process of
pressurizing the Apache Canyon Salt Water Disposal Well No. 19-10
in Las Animas Country, Colorado.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is the use of only known properly working pressure
gauges.

or serious physical harm to employees in that employees were exposed to:
a) On or about September 15, 1989 employees were exposed to the hazard
of substituting welding rods in place of known penny sized nails on
the "Oteco" pressure relief valve located on the "Mud" pump at the
Apache Canyon Salt Water Disposal Well No. 19-10 in Las Animas
County, Colorado.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to use only the manufacturers recommended penny
sized nails in the relief valve which correspond to the desired
relieving pressure.
