Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about 4/25/90, the hazards of explosion and/or fire when allowing aluminum pieces to scrap that are suspected as having uninspected hollows that could contain non-compatible materials in the hollows to be introduced into the re-verb furnace that may cause an explosion or fire. Among other methods, one feasible and acceptable abatement method to correct this hazard is to establish a program that would require an inspection of all aluminum scrap at the time it is received and all suspect hollows be opened and examined.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about 4/25/90, the hazards associated with explosion when scrap aluminum used for charging the reverb furnaces was not preheated to prevent explosions in the furnace and exposing employee(s) to severe burns. Among other methods, one feasible and acceptable abatement method to correct this hazard is to preheat the aluminum prior to allowing it to be charged into the furnace.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about 4/24/90, the hazards associated with explosion and/or fire when using a forklift to charge scrap aluminum into a reverb furnace and the windshield was not sufficient strength to protect operators in the event of an explosion in the furnace. Among other methods, one feasible and acceptable abatement method to correct this hazard is to insure windshields of forklifts used to charge aluminum into furnace are of substantial construction.

Among other methods, one feasible and acceptable abatement method to correct this hazard is to develop and implement a written housekeeping program that would reduce the accumulation of dust in the plant.
