free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazards created by operating the model TC50-1E RO Corporation rubber
tired hydraulic crane (serial number 903) with an employee riding the
main hoist line and caliper clamp out of a trench for a natural gas
pipeline in front of the residences at Rt. 5 Box 926 and Box 927 on or
about 10/21/87.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to assure that employees use a ladder to exit
trenches 4 feet deep or more, or walk up the side if the trench is
sloped enough to accomodate safe egress.
