Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about August 15, 1987, employees were exposed to hazard of being struck by falling tilt-up precast well panels due to the installation of 3/4 inch anchor bolts (expansion bolts) in diagonal braces in that they were not recommended for use by the brace manufacturer since they may not be able to resist the tension and shear forces which accompany panel support. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow recommendations of the brace manufacturer for attachment of braces for tilt-up panels, such as using cast in place inserts or bolts which penetrate through the slab to engage the concrete at top and bottom.
