Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
a crushing hazard caused by two (2) vehicles parked end to end of each
other creating a pinch point due to inadvertent rearward motion of one
of the vehicles:
On August 15, 1988, an employee sustained a crushing injury to the
abdomen while working in the solvent testing bay at the north end of
main tank trailer fabrication building.  The victim was opening a
tanker valve while standing between two M-49 tanker trucks when one
of the truck's engines was started by another employee causing the
truck to roll backwards and pinning the victim between the two
trucks.
One feasible and acceptable abatement method among others to correct
this hazard is to perform a complete hazard analysis of the leak testing
operations; establish work rules and procedures based on theanalysis;
and train each employee involved in the operation in the work rules and
procedures. The work rules and procedures establishedshall include but
are not limited to the following:
1) Set emergency brakes, neutralize transmissions and block wheels prior
to starting engines,
2) Take all precautions necessary to prevent unexpected truck movement
during testing operations,
3) Realign trucks where appropriate to eliminate pinning hazards to
employees(s).
4) Place tags on the truck cab door or the steering wheel indicating
that work is being done on the truck and its engine should not be
started.
