Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) Being struck by large elevator at Stewart Avenue location - The
hoisting doors on the first through the fourth floors ranged in
height from 5'6" to 5'10" high, and hands or other objects could
be placed through the slats on some doors.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to provide hoistway doors which guard the full
height and width of the openings.
