Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On and about June 17, 1987, employees working on the Pre Plodder for line 7 in Department 48 were exposed to hazards of being caught by moving machinery parts such as between the screw conveyor and it's housing in that the equipment was not rendered inoperable prior to performing work. Among other methods, one feasible and acceptable method to correct this hazard is the implementation of an equipment lockout procedure which effectively disengages the machinery from its power sources so as not to be capable of being bypassed and which prevents removal of the locking device by persons other than the one locking the equipment out. Minimum requirements for a lock out program would include: 1) Enforcement of the lock out requirements. 2) Separately keyed locks to assure removal only by the authorized person. 3) Through training on the program for all individuals either required to use the program or required to enforce it.
