Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to electrical shock and electrical burns where employees could
easily damage or contact energized electrical wires, joints,
connections, splices or lugs carrying 110 and 220 volt AC to include
but not limited to the following locations and equipment:
I.   The employees were exposed to contact with live electrical
wires and lugs carrying 110 and 220 volt AC associated
with panelboards in cabinest, cutout boxes, pull boxes
and junction boxes or enclosures:
(a)  main air compressor circuit breaker in the air
compressor room.
(b)  back-up air compressor circuit breaker in the air
compressor room.
(c)  motor controller in the air compressor room.
(d)  sanitation pump motor circuit breaker in the air
compressor room.
(e)  fuel pump motor circuit breaker in the fuel pump room.
(f)  bilge pump motor circuit breaker in the fuel pump room.
(g)  motor controller for fuel pumps in the fuel pump room.
(h)  cooling water pump motor circuit breaker in the fuel
pump room.
(i)  junction box in the entry hall to fresh water room.
(j)  junction box on wall next to refrigeration unit.
(k)  junction box on the refrigeration unit motor.
(l)  junction box for the old indicator lights on the upper
engine room deck.
(m)  outlet box on the aft deck.
(n)  outlet box for aft deck light next to engine room door.
(o)  outlet box in the hallway from the engine room to the
bow storage area.
(p)  outlet box near the work bench.
(q)  outlet box near the starboard door on the upper engine
room deck.
II.   Employees were exposed to contact with energized 110 volt AC
electrical wiring associated with switchboard:
(a)  main control switchboard on upper engine room deck.
III.   Employees were exposed to contact with live 110 volt AC in
flexible cords which could be easily damaged where run as a
substitute as permanent wiring; through holes in the walls,
ceilings, or floors; or through doorways, windows, or
similar openings:
(a)  where flexible cord on aft deck was run into engine
room through the aft engine room door.
(b)  where flexible cord on the aft deck was used on the
aft deck to supply power to receptacle outlets and
aaft running lights.
(c)  where flexible cords were used in the hallway between
engine room and the bow storage area.
(d)  where flexible cords were run through holes in the aft
engine room wall from aft deck.
(e)  where flexible power supply cord for the welding
machine is run through the main electrical panel door.
IV.   Employees were exposed to contact with live 110 volt AC in
flexible cords which were not used in continuous lengths
without splice or tap:
(a)  flexible cord on the aft deck.
V.   Employees were exposed to contact with live 110 volt AC in
flexible cords or the devices they were connected to where
no strain relief is provided and a direct pull is being
applied to the joints and terminations:
(a)  flexible cord connected to outlet box on aft deck.
(b)  flexible cord of the welding machine connected to
circuit breaker termination screws in the main panel
box on upper engine room deck.
(c)  flexible cord connected to lampholder on aft deck and
in hallway from engine room to bow storage area.
VI.   Employees were exposed to contact with noncurrent carrying
metal parts of cord and plug connected equipment when plugged
into outlet receptacles would be connected in reverse polarity:
(a)  receptacle outlet near work bench on upper engine room
deck.
(b)  receptacle outlet near starboard engine room door.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO
CORRECT THIS HAZARD IS:
I.   Provide and install manufacturers covers for circuit breaker
panelboxes, motor switches, outlet and junction boxes, and
fixtures including, but not limited to the following:
(a)  main air compressor circuit breaker in the air compressor
room.
(b)  back-up air compressor circuit breaker in the air
compressor room.
(c)  motor controller in the air compressor room.
(d)  sanitation pump motor circuit breaker in the air
compressor room.
(e)  fuel pump motor circuit breaker in the fuel pump room.
(f)  bilge pump motor circuit breaker in the fuel pump room.
(g)  motor controller for fuel pumps in the fuel pump room.
(h)  cooling water pump motor circuit breaker in the fuel
pump room.
(i)  junction box in the entry hall to fresh water room.
(j)  junction box on wall next to refrigeration unit.
(k)  junction box on the refrigeration unit motor.
(l)  junction box for the old indicator lights on the upper
engine room deck.
(m)  outlet box on the aft deck.
(n)  outlet box for aft deck light next to enginer room door.
(o)  outlet box in the hallway from the engine room to the
bow storage area.
(p)  outlet box near the work bench.
(q)  outlet box near the starboard door on the upper engine
room deck.
II.   Assure switchboard with energized electrical wiring and parts
are only accessible to qualified persons:
(a)  main control switchboard on upper engine room deck.
III.   Install electrical wiring per boat and equipment manufacturers
specifications in a permanent manner:
(a)  where flexible cord on aft deck was run into engine room
through the aft engine room door.
(b)  where flexible cord on the aft deck was used on the aft
deck to supply power to receptacle outlets and aft
running lights.
(c)  where flexible cords were used in the hallway between
engine room and the bow storage area.
IV.   Assure equipment used in wet and damp locations is approved
for that application, including but not limited to the
following:
(a)  receptacle outlet near work bench on upper engine room
deck.
V.   Where flexible cords are connected to devices and fittings
assure that strain relief is provided which will prevent
pull from being directly transmitted to the joints or
terminal screws:
(a)  flexible cord connected to outlet box on aft deck.
(b)  flexible cord of the welding machine connected to
circuit breaker termination screws in the main panel
box on upper engine room deck.
(c)  flexible cord connected to lampholder on aft deck
and in hallway from engine room to bow storage area.
VI.   Assure grounded conductors are installed with the correct
polarity, including but not limited to the following:
(a)  receptacle outlet near work bench on upper engine
room deck.
(b)  receptacle outlet near starboard engine room door.
