access to brake on hoist lift motor on south side of
bridge
ABATEMENT NOTE:  Among other methods, one feasible and acceptable
abatement method to correct this hazard is to install standard
guardrails and toeboards.

climbing onto south girder of North Bridge Crane, and then over
girders, to get onto north footwalk without any fall protection
on or about 6/11/91 at 1:50 p.m.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable
abatement method to correct this hazard is to provide, and ensure
the use of, a vehicle-mounted elevating aerial device which can be
used to access bridge(s) in a safe manner requiring no step over any
gap exceeding 12 inches, and require the use of personal fall
protection.

and/or hot water:
(a)  Power Plant, 6th Floor, #3 Recovery Boiler - At #14 I.K.,
steam and hot water were leaking around packing which had
deteriorated.
(b)  Power Plant, 6th Floor, #3 Recovery Boiler - At #16 I.K.,
steam and hot water were leaking around packing which had
deteriorated.
(c)  Power Plant, 6th Floor, #1 R.B. - Insulation was missing
from hot piping at south ends of #10 I.K., #2 I.K., #4 I.K.,
at Popit Valve(s).
(d)  Power Plant, 6th Floor, #2 R.B. - Insulation was missing
from hot piping at south end(s) of #2 I.K., #4 I.K., #10 I.K.,
#12 I.K., at Popit Valve(s).
(e)  Power Plant, 6th Floor, #2 R.B. - Insulation was missing
from hot piping at north end(s) of #1 I.K., #3 I.K., #9 I.K.,
#11 I.K., at Popit Valve(s).
(f)  Power Plant, 5th Floor, #2 R.B. - Insulation was missing
from hot piping on north end of #19 I.K. at Popit Valve.
