Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) The hazard of unexpected mechanical movement or electrical shock for
employees servicing and maintaining machinery, incorporating the use
of pressurized air, hydraulics and electricty as well as combinations
of these media, as they are not positively locked, blocked or
otherwise secured to deactivate the machines to a zero mechancial
state.
Among other methods one feasible and acceptable method to correct this
hazard is to establish and enforce standard operating procedures which
would include but would not be limited to:
1) Written procedures for performing work on electrical pressurized air,
and hydraulics including:
a) Lockout procedures including assignment of individual locks to all
personnel involved in servicing/maintaining equipment and assigned
supervisory master key control;
b) Training methods of all personnel involved in lockout procedures
invovling electrical, pressurized air and hydraulics.
2) Training procedures for work performed on electrical pressurized air
and hydraulic equipment including:
a) All personnel working in any operations involving the servicing/
maintenance of electrical, pressurized air and hydraulics shall be
trained and informed on any hazards they may encounter on the work
they are to perform; the correct personal protective equipment to be
worn during such work; the methods and frequency of inspection of
personal protective euqipment; and, the proper lockout procedures to
use to assure all electrical disconnects are open and incapable of
being closed/started during course of work performed.
Pressurized air and hydraulic equipment procedures shall address
blocking or securing to ensure a zero mechanicalstate. Training shall
be reviewed on a periodic basis.
b) Supervisor(s) maintaining control of master key(s) shall be trained
on the restrictions on containment and release of such additional
key(s). Supervisors shall also be trained to assure that personal
protective equipment is maintained in good condition and employer's
lockout procedures for electrical, pressurized air and hydraulics
are followed by all employees involved.
