Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling hazards: a) On or about 6-3-89 an employee was allowed and encouraged to participate in the life threatening act of climbing through an 18 inch x 24 inch wall opening, 9 feet above the ground creating the hazard of falls. One feasible and acceptable abatement method, among others, to correct this hazard is to have enforced written policy with every employee trained in such policy, as to the correct procedures to be undertaken in the event they keys to the vault room are locked inside.
