Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that:
a) On or about 12/14/88, and at times prior thereto, at the Cenex
Refinery, Laurel, Montana, employees were exposed to the hazard of
flying debris and fire resulting from failure of the distillate
hydrotreater system in Zone A when the safety relief valve on the
discharge side of the south recycle cylinder of compressor unit
number 3 was rendered inoperative.
One feasible and acceptable abatement method, among others, to correct
this hazard is to assure that safety relief valves are maintained in
proper operating condition when the system is in operation.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that:
On or about 12/14/89, and at times prior thereto, at the Cenex
Refinery, Laurel, Montana, employees were exposed to the hazard of
flying debris and fire resulting from failure of the distillate
hydrotreater system in Zone A due to:
a) Interchanging of compressor cylinders which were not of the same
design nor the identical specific part number as specified by the
manufacturer.
b) Installing parts on compressor units which were not designed or
manufactured by the original equipment manufacturer.
c) Inadequate torquing of fasteners on the compressor cylinder outer
heads.
d) Conversion of compressor cylinders from nonlubricated to lubricated
configurations without consulting the original equipment
manufacturer.
e) Coolant was not installed in a static system of all compressor
cylinder outer heads as required by the manufacturer.
f) Modifying the compressor design by installing plugs in compressor
cylinder water jackets.
g) Pressure gauges were not installed on the discharge side of
compressor cylinders being brought on line in parallel with other
compressor cylinders in operation.
Feasible and acceptable abatement methods, among others, to correct
these hazards are to:
a) Assure that all compressor cylinders are of the same design and the
same identical part number as specified by the manufacturer.
b) Install parts on compressor units which are designed or manufactured
by the original equipment manufacturer.
c) Ensure the adequate torquing of fasteners on the compressor cylinder
outer heads as required by the manufacturer.
d) Consult with the manufacturer prior to converting of compressor
cylinders from nonlubricated to lubricated configuration.
e) Install coolant in the static system of all compressor cylinder outer
heads as required by the manufacturer.
f) Assure that compressors designed for coolant are not modified by
installing plugs in the compressor cylinder water jackets or assure
that any modification of compressor cylinder water jackets is
approved by the equipment manufacturer.
g) Install pressure gauges on the discharge side of compressor cylinders
being brought on line in parallel with other compressor cylindersin
operation.
