Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazard of crushing injuries while performing maintenance or repair to automatic pin setters. On or about October 16, 1988 employee(s) attempted to repair an AMF Pin Setter without electrically locking the machine out. Among other methods one feasible and accepted method to correct this hazard is to establish and enforce standard operating procedures to reduce equipment to a zero state of energy during repair/maintenance work (electrical lockout).
