Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) The hazard of unexpected mechanical movement or electrical shock for
employees servicing and maintaining machinery, incorporating the use
of pressurized air, hydraulics, and electricity as well as
combinations of these media, as they are not positively locked,
blocked or otherwise secured to deactivate the machines to a zero
mechanical state. Two employees working on the c-press auger changing
a shaft on the hydraulics located in the SBC 2nd floor Mezzanine did
not deactivate the auger to a zero mechanical state.
Among other methods, one feasible and acceptable method to correct this
hazard would include but not be limited to:
1) Include in the written policy as well as in practice the assignment
of individual locks to be used by all persons involved in servicing
and/or maintaining equipment assigned supervisory master key control.
2) Enforce the lockout rule that each person servicing or maintaining
equipment place each individual lock on the electrical equipment
being deactivated, and the supervisor strictly adhere to the
enforcement of the policy.
b) The hazard of employee entanglement in rotating parts while
maintaining or cleaning in and around operating machinery without
deactivating and locking out the energy source.
Among other methods, one feasible and acceptable method to correct this
hazard would include but not be limited to:
1) Enforcing the established written lockout policy whenever employees
are required to clean in and around the operation Grain Dryers in
the SBC and NBC.

or serious physical harm to employees in that employees were exposed to:
a) The hazard of employees becoming exposed to hazardous vapors as a
result of incompatible chemicals stored together, on the 7th floor
of the NBC.
Among other methods, one feasible and acceptable method to correct this
hazard would be to separate the 3 barrels of Corrosive Foster Acid from
the 19 barrels of Hypochlorite solution laundry bleach by 100 feet or
provide separate approved chemical storage rooms.
