Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed to: the
hazard of oxygen deficiency resulting in asphyxiation and/or death.
a)  Out Outside Plant;
Maintenance employee(s) enter into 7 ft. deep by 24" wide pit to check
on water valves.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is:
1.  Provide and ensure the equipment necessary for safe entry into the
pit including atmospheric testing for oxygen and any combustible or
flammable gas.
2.  Provide rescue equipment, such as emergency respirator, retrieval
lines, bye line and a standby person outside the pit.
3.  Provide training to the employees on the hazard that may be faced
during entry into the pit, communication with attendents use of personal
protective equipment and rescue  procedures.
4.  Provide mechanical ventilation prior to entry and during occupancy
of the confined space.
5.  Provision of a confined space entry permit.
