Section 5(a)(1) of the Occupational Safety
and Health Act of 1970: The employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees
in that employees
were exposed to:
a)  Employees were exposed to the hazard of being caught and/or crushed
between the elevator
and shaftway wall or sills.  A car door or gate was not provided at the
entrance of elevator car.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to:
Immediately install a car door or gate at the car entrance and equip
the car door or
gate with an electric contact.  Reference Publication, 1.  American

elevator without a means
of a working emergency alarm system during an emergency such as a fire
injury, technical
failure, etc.  The emergency signal button inside the elevator car to
alert personnel of an
emergency was not working. Among other methods, one feasible and
acceptable method to
correct this hazard is to:
Repair or replace existing emergency signal button.  Reference
