Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) Employees were exposed to hazards inherent to mechanical motion of
the chain and sprocket drive, while unjamming barrels on the conveyor
located next to the blaster as a result of not being rendered
inoperable during that procedure.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to establish and enforce an adequate plant
lockout procedure which would include the essentials:
1) Ensuring that all persons involved in operations where lockout is
required receive instructions and training in lockout procedures.
2) Ensuring that equipment is rendered safe (either by the use of
electrical equipment or by attempting to activate the equipment); and
3) Ensuring that regular enforcement and periodic evaluation of lockout
practices are made to determine usage and adequacy of the procedures.
The Queen City Barrel Company was previously cited for a violation of
this Occupational Safety and Health Standard of its equivalent standard
5(a)(1) which was contained in OSHA Inspection No. 101647220, Citation
No. 3, Item No. 1, Issued on December 21, 1986.
