Section 5(a)(1)of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) The employer did not furnish employment which were free from
recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that on or about August 6, 1987
employees were exposed to possible electric shock or electrocution
while installing buss duct plugs onto the 440 volt buss duct in the
single stage Machining area without locking out and tagging the main
disconnect switch in the Power room near the southeast corner of the
building as the result of not enforcing a lockout/tagout procedure
and making periodic evaluation of the procedure to determine the
adequacy of the procedure.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is, but nto limited to:
1) Ensure that locks keyed separately and/or multiple locking devices be
provided and used when more than one person is involved in operations
requiring a lockout;
2) Ensure that equipment is rendered safe, either by the use of
electrical test equipment or attempting to activate the equipment;
3) Training of all employees involved in implementing the program;
4) Ensure that regular enforcement and periodic evaluation of the
lockout/tagout procedure is made to determine the adequacy of the
program.
