Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
crushing injuries or death while working below elevated cars and trucks
on a defective car lift which could allow vehicles to fail or lower
unexpectedly:
On the Rotary Model SM-50 HR automotive lift, one support roller at the
end of the rolling bridge was displaced inward so that the shaft end was
bearing on the edge of the supporting H-beam lower flange.
This condition could cause excessive wear or deformation of the shaft
and/or flange and could result in bridge failure.
One feasible and acceptable method to correct the hazard, among other
methods, is to restore the support roller to its proper position,
replace worn or damaged parts and institute a program, of regular
inspections, maintenance and employee training to ensure that any
damage or defects is immediately corrected.
