Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
The hazards of crushing injuries, broken bones, and death while
working, walking, or standing in the area where overhead pipe was
being cut off and allowed to fall uncontrolled to the floor. On or
about October 18, 1986, in the L-2 and L-3 aisleway, the pipe section
being removed was not restrained to prevent its falling, and
employees were not restricted from entering the hazardous areas.
Among other methods, one feasible and acceptable method to correct this
hazard is to identify and restrict access to the hazardous area by
barricades, rope, warning signs, or similar devices. The hazard can be
further minimized by securing the section of pipe to be removed with
material handling equipment capable of safely lowering the pipe to the
floor.
